Lyle Bohlman, M.D.

Program Director (Family Medicine)

Dr. Bohlman Graduated from the University of Oklahoma in 1974 and trained in Family Medicine at Saginaw Collaborative Hospitals in Saginaw, Michigan. He has more than 45 years’ experience practicing medicine. He spent the first 11 years of practice in rural Oklahoma, New Mexico and Montana doing full-service Family Medicine including OB. He then moved to Boston, bought an urgent care center, and was soon recruited into Family Medicine teaching where he became the associate program director at Tufts University Family Medicine residency.

Rodney Holladay, M.D.

Program Director (Internal Medicine)

Dr. Rodney Holladay received his Doctorate of Medicine from the University of Tennessee Health Sciences Center in 1982. Dr. Holladay is board certified in Internal Medicine and has special interest in Geriatric Medicine. He has participated in multiple private practice settings, and prior resident and medical student teaching sites including inpatient hospital services, outpatient clinics, long-term care facilities and rehabilitation units.

Jeremy Johnson, M.D.

Associate Program Director (Family Medicine & Emergency Medicine)

Dr. Jeremy Johnson attended medical school at The University of South Alabama. He completed his residency in family medicine at the UAB Heersink School of Medicine Huntsville Family Medicine Residency Program and completed additional training in emergency medicine at the University of Tennessee Health Sciences Center Jackson. He is board certified in both Family Medicine and Emergency Medicine. In addition, he completed a faculty development fellowship with the UAB Heersink School of Medicine Department of Family Medicine.

Saketh Shekar, M.D.

Associate Program Director, Internal Medicine & Subspecialty Education Coordinator (Pulmonary & Critical Care Medicine)

Dr. Saketh Palasamudram Shekar is an Interventional Pulmonologist board certified in Internal Medicine, Pulmonology and Critical Care. He completed his residency at the Atlantic Health Center in New Jersey and went on to complete a Pulmonary and Critical Care Fellowship at the Cleveland Clinic. In addition, he has also completed a sub-specialty in Interventional Pulmonology at NYU.

Mahvash Mozafarian, M.D.

Associate Program Director

Dr. Mozafarian is an ABIM board certified internist at Crestwood Medical Center. She completed her internal medicine residency at MedStar Health in Baltimore, MD, and North Alabama Medical Center in Florence, AL.

Adeel Bodla, M.D.

Dr. Adeel Bodla is a native of Multan, Pakistan. He attended NMC, Multan Pakistan. Dr. Bodla joined the Crestwood Family Medicine Residency group due to his longtime association with Crestwood Hospital. During his free time, he enjoys badminton, golf, hiking and tennis. Dr. Bodla hopes to continue to constantly provide the best medical care to the community, now passing it on to the next generation of residents.

Jeffrey Garber, M.D.

Dr. Jeffrey Garber obtained his medical degree from the University of Louisville in his hometown of Louisville, KY. Dr. Garber joined Crestwood Medical Center due to its reputation for being an all-around great hospital. He has a huge passion for aviation. Dr. Garber hopes to leave an impact on the medical field by increasing the number of capable family physicians in our community.

Darla Cowart, M.D.

Dr. Darla Cowart obtained her Doctorate of Medicine at UAB School of Medicine. She went on to complete a Family Medicine Residency and Sports Medicine Fellowship at UAB Family Medicine in Huntsville, AL. She is board certified in both Family Medicine and Sports Medicine.

Vanessa Henri, M.D.

Dr. Vanessa Henri is a native of Miami, Florida and completed her undergraduate degree in biomedical sciences at Oakwood University in Huntsville, Alabama. She attended medical school at Loma Linda University, and residency at the University of Miami/Jackson Memorial Hospital’s Internal Medicine and Pediatrics residency program, where she also served as Chief Resident.

Ali Hassoun, M.D.

Core Faculty & Subspecialty Education Coordinator (Infectious Diseases)

Dr. Ali Hassoun is a Clinical Professor of Medicine. He is an infectious diseases specialist, certified by the American Board of Internal Medicine and Infectious Diseases. He graduated from University of Baghdad school of Medicine and trained with the Royal Australian College of Physicians, Auckland, New Zealand and completed his fellowship at the Long island Jewish Medical Center, New York. He is the chairman of the Infection Control Committee at Crestwood Medical Center.
